ABOUT US
We are passionate about reclaiming the dignity in healthcare and empowering health consumers.
We believe the days of receiving and providing fragmented care should be over. The old adage of "it takes a village" is the model how healthcare should be delivered. Education, prevention and team-based care is how health and well-being are produced for our families and communities. A healthy community is a thriving community.
As consumers of health services we saw that there had to be a new way where the Primary Care Physician is empowered to educate and help the patients make better decisions. A new way that takes a whole-person perspective (body, mind, and soul) and incorporated the culture in which he or she lives and its social and intellectual dynamics. We have seen how team approaches and group processes that incorporated the faith and emotional strengths of the patients produced greater health and well-being.
We have gained the trust and respect of the community by doing the right thing for them and emphasizing education, prevention and access to quality care. This led us to small and medium size employers who wanted to also do the right thing for their employees and their families. They were seeing the toll that chronic disease and high-risk health behaviors were putting on the employees and the negative results on productivity. And they were looking for a solution.
Thus, for their employees and patients we have developed online courses, campaigns and processes of behavior change that are culturally-relevant and encompassing the four areas of well-being:
Together we are changing the lives of employees, patients, and community. We are changing their perceptions (their thought processes and belief system, full of hopes and also fears), their emotions (how their feelings, wants and needs interact) to change their actions (their behaviors and decisions) for better health and true well-being.
Our models are based on science (Dr. Prochaska's stages of change, motivational interviewing, group therapy, interpersonal relationships...) so that we patients, employees, physicians and employers can become the best versions of themselves, as God intended us to be.
